Submissions

1. Print a submission form and fill out all customer information, including your customer ID. If you do not have a customer ID, please e-mail us and we will issue you one.
2. Fill in the box that says "Form Number" with your last name, followed by the date.
   Example: John Smith      Form #: Smith5/15/07
3. Place each coin in a plastic 2x2 flip along with an insert. One each insert, please place the form number and the corresponding line number for that coin.
4. Calculate the total amount due and send the coins, submission form and payment to:

PLEASE ONLY SEND SUBMISSIONS VIA USPS REGISTERED MAIL!

We accept: Money Orders, Cashiers Checks, Personal Checks and Business Checks.

All submissions must be paid in full before your order will be processed.

For multiple submission forms, please fill out the form number as described above and follow the date with A, B, C, etc.

Form number: Smith5/15/07A
